Nutrición artificial en el paciente críticamente enfermo: la estrategia del soporte metabólico

Abstract

La nutrición artificial fue exitosamente iniciada hace más de un cuarto de siglo. Desde entonces ha sido constante preocupación, perfeccionar y mejorar las maniobras terapéuticas, para obtener cada vez un mayor beneficio. La mayoría de los pacientes sobrevive a un episodio de stress, pero repetidos insultos por anestesia y cirugía, a menudo tienden a prolongar la enfermedad con complicaciones, una de las cuales, la malnutrición puede adicionar mayor morbilidad y mortalidad de esos pacientes. El propósito de este trabajo es revisar los mecanismos por los cuales el paciente crítico se desnutre y las consecuencias que esto tiene, y establecer algunos criterios para el inicio del soporte nutricional.

Since this time, it had been worring constant to perfect and to improve the maneuvers therapeutics, to get each time a more benefics. Most patients survive a single episode of stress, but repeated insults by anesthesia and surgery often lead to prolonged illness with complications, one of which, malnutrition, can add substantially to the morbidity and mortality of these patients. It is the purpose of this paper to review the mecanisms because the critically ill be malnourished and the consecuence that it has and to stablish some criteriousfor the start the nutrional support.
