La prueba de fijación del complemento en el diagnóstico de la histoplasmosis

Abstract

Se investigaron anticuerpos fijadores del complemento frente a los antígenos de Histoplasma CapsuIatum, en el suero de 72 pacientes con síndrome respiratorio agudo y crónico, con signos anormales en radiografía de tórax, baciloscopia negativa y tratamiento antituberculoso. Resultaron 6 (8%) fijadores del complemento, 6 (8%) anticomplementarios y 60 (84%) negativos. Dos pacientes mostraron títulos >1:4 frente al antígeno micelial (AgM) y negativo al antígeno levaduriforme (AgL) y 4 pacientes mostraron títulos < 1:8, frente a los dos antígenos. Un caso fue considerado como posible histoplasmosis "altamente sugestivo" y otro como histoplasmosis diseminada con evidencia histopatológica y cultural de histoplasmosis extrapulmonar. La edad de los pacientes de sueros positivos varió de 20-49 años y todos residieron temporalmente en zonas endémicas. Se comenta la utilidad clínica de los resultados y pruebas serológicas comunes.

Resulting complement fixation test positive 6 (8)/0), anticomplementaries 6 (80/0) and negatives 60 (84%). Two patients showed titers < 1:4 to the micelial antigen (AgM) and negative to the yeast antigen (AgL) and four patients showed titers > 1:8 to the two antigens. One case was considerated as possible histoplasmosis "highly, suggestive” and other as disseminated infection with histopatologic and cultural evidence extrapulmonary histoplasmosis. These patients were predominately between the ages of 20 and 49 years old and all temporally resided in endemic zone. It's coment the availability clinic of the results and currents serologic tests.
